The Consumer Directed Personal Assistance Program (CDPAP) is an alternative to traditional home care services. It is a self-directed, Medicaid home care program available to eligible consumers who are in need of home care services and are capable of directing their own care.

Home Health Aide

HHA stands for Home Health Aide in the context of home care services in the United States. A Home Health Aide is a trained professional who provides assistance and support to individuals in their homes, helping them with activities of daily living (ADLs) such as bathing, dressing, grooming, meal preparation, medication reminders, and light housekeeping. Home Health Aides often work under the supervision of a registered nurse or other healthcare professional to ensure that the client’s healthcare needs are met. They play a vital role in enabling individuals to receive care and support while remaining in the comfort of their own homes.

The Nursing Home Transition and Diversion Waiver

(NHTD) is a Medicaid program that provides home and community-based services to eligible individuals who would otherwise require nursing home care. This program aims to help individuals with disabilities or chronic health conditions live independently in their own homes or community settings. Services may include personal care, skilled nursing, occupational therapy, physical therapy, and home modifications. NHTD participants work with a care manager to develop a care plan that meets their individual needs and goals. The program is designed to be cost-effective, as it is less expensive to provide services in the community than in a nursing home.

Traumatic Brain Injury

(TBI) is a type of brain damage caused by a sudden and violent blow or jolt to the head or body. This injury can result in a range of physical, cognitive, and emotional symptoms, such as headaches, dizziness, memory loss, difficulty concentrating, mood swings, and depression. TBI can be caused by a variety of incidents, including falls, car accidents, sports injuries, and assaults. Treatment for TBI may include rehabilitation therapies, medication, and surgery, depending on the severity of the injury.

Consumer Directed Personal Assistance Program

CDPAP
