How the NHTD Program Supports Individuals with Dysphagia 

The NHTD waiver program provides a multifaceted approach to managing individuals with dysphagia. They create a safe and effective nutrition plan for them with the help of various professionals in treating and managing dysphagia patients. This support is implemented in your Individualized Service Plan (ISP) and delivered through several key services.  

Skilled Nursing Services:  

A registered nurse (RN) usually helps in monitoring the participant’s clinical needs, including those that are related to nutrition and swallowing. This service will be given in the following steps: 

Clinical Assessment: An RN will perform an assessment of the participant’s swallowing ability, monitor for signs of aspiration (when food or liquid passes through the food pipe into the stomach), and evaluate their nutritional status.  

Care Plan Development: The nurse will work with the participant’s physician and other therapists to develop a comprehensive care plan. This plan may include dietary guidelines, safe swallowing techniques, and emergency protocols.  

Education: The nurse will educate the participant, their family, and the Home Health Aide on topics like identifying the signs of choking or aspiration, proper positioning during meals, and preparing foods to eat at the same time.  

Management of Feeding Tubes: Individuals who need enteral feeding a method of delivering nutrition and calories directly into the gastrointestinal tract), the skilled nurse will manage the tube, provide feeding, and keep the site clean and free of infection.  

 

Speech-Language Pathology (Speech Therapy) 

A Speech-Language Pathologist (SLP) is assigned to diagnose and treat your swallowing disorders. This service plays a crucial part in dysphagia management within the NHTD program. It includes:  

Swallowing Evaluation: An SLP will conduct a swallowing evaluation, which sometimes involves an instrumental assessment like a Modified Barium Swallow Study (MBSS), to precisely check the nature of the swallowing problem.  

Dietary Recommendations: Based on the evaluation results, the SLP will recommend the safest foods and liquids to consume. This may include minced and moist foods, pureed diets, or thickened liquids.  

Therapeutic Exercises: The SLP will teach you some exercises to strengthen the muscles involved in the swallowing process.  

Compensatory Strategies: The therapist teaches you some strategies to make swallowing safer, such as the chin tuck technique or taking small, deliberate bites. 

 

  1. Home Health Aide (HHA) Services:

These services ensure your daily hands-on support on personal care with a nutrition plan and safety. It includes: 

Meal Preparation: The HHA is trained to prepare meals according to your requirements based on the recommendations made by the SLP. This includes pureeing foods or adding thickeners to liquids as prescribed.  

Assistance with Feeding: The HHA assists patients who can’t consume their food, ensuring they sit correctly and take time to chew and swallow.  

Monitoring and Reporting: This aid is given to ensure that any signs of distress, coughing, and choking during meals do not go undetected and sends its reports directly to the supervising nurse or service coordinator for further action.  

 

Creating a Person-Centered Nutrition Plan 

To create a person-centered nutrition plan, your service coordinator will work with you, your family, and the entire clinical team to develop the nutritional plan that meets your needs. The process involves the following steps:  

Identifying Needs: During the initial assessment for care planning, your services coordinator will discuss your difficulties with eating and swallowing to understand your problem.  

Incorporating Specialist Care: The coordinator then arranges evaluations from an SLP and ensures that RH will examine your condition and help in developing the care plan.  

Authorizing Services: Once your Individualized Service Plan (ISP) is finalized, with the required services like therapies, nursing services, and specific instructions for your HHA. The established NHTD rates are discussed with approved providers to ensure the delivery of these essential services. This collaborative approach ensures that everyone on the care team works together to provide consistent and safe support for every meal.
