You Have the Right to Choose Your Provider
The New York State Nursing Home Transition and Diversion (NHTD) and Traumatic Brain Injury (TBI) Medicaid waivers are built around participant choice. You can decide which approved agencies provide your waiver services, and you can ask to change them if your needs aren’t being met. You don’t need to give a reason or “prove” a problem. Your comfort, safety and independence come first.
Your Service Coordinator is your partner in this process. They help update your Service Plan so your new provider is approved and your services continue.

How the Transfer Works
Switching agencies is easier than most people expect. Here’s what happens:
1. Call HouseCalls
Tell us you're on NHTD or the TBI waiver and want to transfer. We'll answer your questions and explain what's next.
2. Tell your Service Coordinator
Let your Service Coordinator know you want HouseCalls as your new HCSS provider. If it helps, we can join the conversation.
3. Your Service Plan is updated
Your Service Coordinator revises your plan and submits it for approval through the Regional Resource Development Center (RRDC).
4. We get to know you
Our team meets with you to learn your routine, needs and preferences, and to match you with the right aide.
5. Care starts on a set date
We coordinate the start date so your new services begin when your old ones end, with no gap in care.

Who We Serve
HouseCalls supports NHTD and TBI waiver participants across New York City. You may qualify for waiver services if you:
- Have Medicaid coverage
- Need a nursing home level of care but want to live at home
- Are 18 or older with a physical disability or traumatic brain injury
- Are 55 or older with cognitive decline

Frequently Asked Questions
Can I change my NHTD or TBI home care agency?
Yes. Waiver participants have the right to choose their service providers and to request a change at any time. Your Service Coordinator will help update your Service Plan.
Will my services stop while I transfer?
No. We coordinate the start date with your Service Coordinator so your new care begins when your current services end.
Do I have to tell my current agency I'm leaving?
Your Service Coordinator handles the provider change through your Service Plan. You don’t have to explain your decision to your current agency.
Does transferring cost anything?
No. Your waiver services are covered by Medicaid, and changing providers doesn’t change that.
How long does a transfer take?
It depends on your Service Plan approval, but our team moves quickly and keeps you updated at every step.
Can I keep my current aide?
If your aide is interested in joining HouseCalls, talk to us. We’ll let you know what’s possible.
Do I also need to change my Service Coordinator?
No. You can keep your Service Coordinator and change only your home care provider, or change both. It’s your choice.
