When to Consider These Programs
At HouseCalls Home Care, we offer Medicaid Waiver Programs including NHTD (Nursing Home Transition and Diversion) and TBI (Traumatic Brain Injury), helping individuals with cognitive or physical disabilities stay safe and supported at home.
Eligibility Requirements
You must meet one of the following:
- Have Medicaid coverage
- Need nursing home level of care at home
- Be 18+ with a physical disability or traumatic brain injury
- Be 55+ with cognitive decline
NHTD & TBI Programs
A CDPAP caregiver can assist with:
Transitions from nursing homes
Support for individuals with brain injury
HCSS – Home and Community Support Services
