What Does New York Medicaid Coverage Include for Home Care?

New York Medicaid is the primary funding source for long-term home care in the state, serving more than 6.4 million enrollees statewide, according to the New York State Department of Health. For those who qualify, it can fund consistent, ongoing assistance at home.

The most common benefit is Personal Care Services (PCS), delivered by a trained aide employed through a licensed agency. Coverage typically falls into two levels:

  • Level 1 Activities: Laundry, meal preparation, grocery shopping, light housekeeping, and medication reminders
  • Level 2 Activities: Bathing, dressing, grooming, transferring, toileting, walking, and feeding

Medicare does not cover long-term home care. It may pay for a short skilled nursing stay or a limited number of aide visits after hospitalization — but it is not designed for ongoing support at home. Medicaid is.

Which Programs Deliver Medicaid Home Care Services in New York?

New York offers several pathways to home care services under Medicaid:

  • Managed Long Term Care (MLTC): The primary route for most recipients. An MLTC plan coordinates with licensed home care agencies to provide aides on an ongoing basis.
  • Consumer Directed Personal Assistance Program (CDPAP): This program allows your loved one — or a designated representative — to hire and direct their own caregiver, including adult children or other trusted family members, who can be paid for their care. CDPAP aides may also perform certain skilled tasks, such as insulin injections, that standard aides cannot.
  • Certified Home Health Agencies (CHHAs): These agencies provide short-term, skilled services such as nursing or physical therapy — typically after a hospital or rehabilitation discharge.

Who Qualifies for Medicaid Home Care Coverage in New York?

Qualifying for Medicaid does not automatically mean qualifying for home care. Starting September 1, 2025, adults newly applying for Personal Care Services, CDPAP, or MLTC enrollment must meet a minimum Activities of Daily Living (ADL) test — a higher eligibility bar than previously required.

What Are the Income and Asset Limits for Medicaid Home Care?

If your loved one’s income exceeds the monthly limit, surplus income can often be directed into a pooled income trust — which effectively reduces countable income to within the eligible range. Exceeding the threshold does not automatically disqualify someone from coverage.

How Do You Start Accessing Medicaid Home Care Services?

The process begins with contacting the New York Independent Assessor Program (NYIAP) at 855-222-8350. NYIAP conducts two assessments — a Community Health Assessment and a Clinical Appointment — to determine whether your loved one qualifies for MLTC enrollment and meets the medical criteria for home care.

Once approved, your loved one selects an MLTC, MAP, or PACE plan. That plan then develops a care plan and determines the number of hours of care that are medically necessary. If a request is denied at any point, your family has the right to appeal through a Fair Hearing.
