If you live in New York and are looking into waiver programs for yourself or a loved one, it is easy to feel confused. The NHTD waiver and the TBI waiver have a lot in common. Both are designed to help eligible people receive services in the community instead of in an institution. Both also focus on independence, safety, and person-centered planning.
Still, they are not the same program. The biggest difference is who each program is meant to serve and which services are unique to each one.
What Is the NHTD Waiver?
The NHTD waiver stands for Nursing Home Transition and Diversion. It is a New York Medicaid program for eligible adults who need nursing home-level care but want to live in the community. It serves adults ages 18 to 64 with a verified physical disability and adults aged 65 and older who meet the program requirements.
Its main goal is to help people avoid nursing home placement or transition out of a nursing home and live more independently at home or in another community setting.
What Is the TBI Waiver?
The TBI waiver is for eligible individuals with a traumatic brain injury who need specialized community-based support. Like NHTD, it is designed to help people live outside of institutional settings. However, the TBI waiver is built around the unique needs that can follow a brain injury, including behavioral, cognitive, and rehabilitation-related challenges.
What Both Programs Have in Common
According to the New York State Department of Health, both programs include many similar services. Shared services include:
- Service Coordination
- Assistive Technology
- Community Integration Counseling
- Community Transitional Services
- Environmental Modifications
- Home and Community Support Services
- Independent Living Skills Training
- Positive Behavioral Interventions and Supports
- Respite Services
- Structured Day Program Services
Both programs are also managed locally through Regional Resource Development Centers, or RRDCs.
NHTD-Only Services
Some services are available only through NHTD. These include:
- Congregate and Home-Delivered Meals
- Home Visits by Medical Personnel
- Moving Assistance
- Nutritional Counseling and Educational Services
- Peer Mentoring
- Respiratory Therapy Services
- Wellness Counseling Service
These services can be especially helpful for older adults and people with physical disabilities who need practical support to remain safe at home.
TBI-Only Services
The TBI waiver includes certain services that are not part of NHTD. These include:
- Substance Abuse Program Services
- Waiver Transportation
These services reflect the more specialized needs that may come with traumatic brain injury.
Who May Qualify?
The NHTD waiver generally applies to people who:
- Are eligible for community-based Medicaid
- Need a nursing home level of care
- Are ages 18 to 64 with a verified physical disability, or age 65 or older
- Can live safely in the community with supports
The TBI waiver also serves people who need community-based supports, but it is specifically intended for individuals with traumatic brain injury. In other words, the person’s diagnosis and needs help determine whether TBI is the right fit.
How to Think About the Difference
A simple way to understand it is this:
- NHTD is mainly for eligible adults with physical disabilities or older adults who need nursing home-level care.
- TBI is for eligible people whose needs are tied to a traumatic brain injury and who may benefit from specialized brain injury-related services.
So even though the programs overlap in many ways, they are designed for different populations.
Why the Right Program Matters
Choosing the right waiver matters because services, eligibility, and care planning can vary. The best fit depends on the person’s medical condition, daily support needs, and long-term goals. Families should look closely at both eligibility rules and the list of services before making a decision.
